provide you with an itemized statement which you may forward
to your insurance company for reimbursement. Please note that
you are ultimately responsible for your bill and we appreciate
and encourage payment for visits and elective
surgery at the time of your visit. For your convenience we
accept cash, personal checks, MasterCard and Visa.
We
will bill your insurance company directly for non-elective
surgery and Medicare covered services. We participate with
Medicare, Blue Cross/Blue Shield, Priority Health, BlueCare
Network and other major insurance programs. If you have
questions regarding insurance coverage, do not hesitate
